Within 30 days of one the following events, <br /> business shall electronically update the information submitted to the California Environmental Reporting System <br /> (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Documentation of employee training was not available at the time of inspection. The business plan shall include <br /> provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> This is a Class II violation. <br /> 15 HSC 25510(a) Failed to report actual or threatened release to the EHD and/or CalOES Warning Center. <br /> A threatened release of paint, epoxy, and other unknown type material involved in a fire (type and quantity unknown <br /> at this time due to safety and accessibility issues)occurred on August 4, 2018 and was not reported to the EHD and <br /> CalOES. A release or threatened release of a hazardous material shall be verbally reported immediately to the EHD <br /> and CalOES. Ensure that any future releases or threatened releases of a hazardous material are verbally reported <br /> immediately to the EHD and CalOES. <br /> This is a Class I violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the completed inspection report. <br /> Assistance with CERS access and submittal was provided at the time of inspection. <br /> Contact the EHD when the trailers parked along the north and east areas of the facility are accessible for <br /> inspection. <br /> Business advised of mandatory release and threatened release reporting requirements. <br /> FA0009455 PR0520928 SCO01 08/17/2018 <br /> EHD 19-01 Rev.10/31/2017 Page 4 of 5 Hazardous Materials Program OIR <br /> 1868 E.
