S A 1 Y J 0 a Q U I N Environmental Health Department <br /> Hazardous Materials Program Inspection Report <br /> Facility Name : Facility Address : Date : <br /> JC TRUCKING 1415 W ANDERSON ST , STOCKTON August 17 , 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I , CLASS II, or MINOR - Notice to Comply) <br /> Item # Remarks <br /> 4 HSC 25508 ( a ) ( 1 ) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page . <br /> The Business Owner/Operator Identification form was not submitted or complete . John Cervantes has not been the <br /> owner of this business since at least December 2017 . This information must be current and submitted as part of the <br /> business plan . Immediately log into the California Environmental Reporting System ( CERS) at <br /> http : //cers, calepa . ca . gov/ , enter the correct or updated information , and submit to the EHD for approval . <br /> This was corrected on site . <br /> This is a Class II violation. <br /> 5 HSC 25506 (a ) ( 1 ) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site . <br /> At least 155 gallons of paint , probable reportable amount of soda flake ( possibly 12 cans ) , and many containers ( 55 <br /> gallon drums , and cans) of other unknowns that are not safely accessible , are currently being stored in a fire <br /> damaged trailer, were found on site that have not been reported . Any material that meets or exceeds the <br /> reportable quantity shall be reported in the facility ' s business plan . Immediately log into the California Environmental <br /> Reporting System ( CERS) at http : //cers. calepa . ca . gov/, add the materials to the hazardous material inventory, and <br /> submit to the EHD for approval . <br /> This is a Class II violation. <br /> 6 HSC 25505 (a) (2 ) Failed to complete and /or submit a site map with all required content . <br /> The map did not show loading areas , internal roads , access and exit points , emergency shutoffs , hazardous <br /> material handling and storage areas ( paint in trailer and shop ; propane tank has been replaced by propane <br /> cylinders) . A site map shall contain a north orientation , loading areas , internal roads , adjacent streets , storm and <br /> sewer drains , access and exit points , emergency shutoffs , evacuation staging areas , hazardous material handling <br /> and storage areas , and emergency response equipment. If a site map element is not applicable for your facility then <br /> list it on the map and label as " NA" .
