I Emergency Services. Further, it is alleged that said violation was knowingly committed after <br /> 2 reasonable notice; <br /> 3 e. Violation of Health and Safety Code §25505(e)(2), failed to annually submit a <br /> 4 form, provided by the San Joaquin County Office of Emergency Services, that certifies that the <br /> 5 handler has included, and maintains as current, in the business plan, all other information required by <br /> 6 Health and Safety §25504. <br /> 7 4. Defendants with actual or constructive knowledge of this injunction, only in so far as they <br /> 8 are doing business in the State of California, in the course of defendant's COMMERCIAL <br /> 9 DISHWASHING LEASING activities, are pursuant to Business and Professions Code § 17204 <br /> 10 hereby permanently enjoined from: <br /> 1 I a. Violation of Chapter 6.95, of the Health and Safety Code described in paragraph 3 <br /> 12 - 3e above. <br /> 13 5. Defendants shall pay to plaintiff the sum of ONE THOUSAND SEVEN HUNDRED <br /> 14 FIFTY DOLLARS ($1,750.00), as and for civil penalties, pursuant to Business and Professions Code <br /> 15 §§ 17200 et seq.. Said payments shall be made Payable to the TREASURER OF SAN JOAQUIN <br /> 16 1 COUNTY and delivered to the San Joaquin County Office of the District Attorney, Environmental <br /> 17 Prosecutions Unit. <br /> 18 6. Defendants shall pay to plaintiff the sum of ONE THOUSAND SEVEN HUNDRED <br /> 19 FIFTY DOLLARS ($1,750.00), as and for civil penalties, pursuant to Health & Safety Code §§ <br /> 20 25514 and 25515.2. Said payment shall be due and payable to the following in accordance with <br /> 21 Health and Safety Code §§ 25514 and 25515.2: <br /> 22 San Joaquin County Office of the District Attorney$875.00 <br /> 23 San Joaquin County Office of Emergency Services $875.00 <br /> 24 Said payments shall be delivered to the Office of the San Joaquin County Office of the District <br /> 25 Attorney, Environmental Prosecutions Unit. <br /> 26 7. Defendants shall pay to plaintiff the sum of THREE HUNDRED DOLLARS ($300.00), <br /> 27 for partial cost of investigation incurred by the San Joaquin County Office of Emergency Services. <br /> 28 3. <br />
