The following is an itemized list of hazardous materials violations that have not been <br /> addressed for QUEST INDUSTRIES LLC as of June 01, 2017. <br /> Open violations from April 25, 2017 inspection <br /> Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> Reportable amounts of propylene glycol and cleaner degreaser were found on site that have not been reported. Any <br /> material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately <br /> log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to <br /> the hazardous material inventory, and submit to the EHD for approval. <br /> The amount of water base paint present at this facility is closer to 2,400 gallons vs. the 60 gallons currently reported <br /> in the inventory. This information must be current and submitted as part of the business plan. Immediately log into <br /> the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> Violation#6 - Failed to complete and/or submit a site map with all required content. <br /> The site maps do not show the actual storage and handling locations of the propane, water base paint, waste water <br /> base paint, propylene glycol, waste powder coating and the cleaner degreaser. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> Violation#7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were lacking or inadequate. The phone number for the EHD/CUPA was not <br /> identified. A business plan shall include the following emergency response procedures for a release or threatened <br /> release of hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation #10 -Failed to submit revisions to business plan within 30 days of substantial change in <br /> operations. <br /> During the inspection, it was found that the amount of water base paint present at this facility is closer to 2,400 <br /> gallons and reportable amounts of propylene glycol and cleaner degreaser were present and the updated <br /> information has not been submitted. Within 30 days of one the following events, business shall electronically update <br /> the information submitted to the California Environmental Reporting System (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation#11 -Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD.
