HAZELTON AVENUE <br /> STOCKTON, CALIFORNIA 95205 <br /> RE: RETURN TO COMPLIANCE <br /> On October 301h 2018 our facility located at 1549 N. Broadway was visited by Robert Lopez HazMat <br /> Specialist II for a random inspection of our facility and hazardous material business plan. During that <br /> visit there were violations discovered as outlined in the Summary of Violations Hazardous Materials <br /> Program Inspection Report. <br /> HSC 25505(a)(1) Failed to complete and/or submit hazardous material inventory for all reportable materials on <br /> site. <br /> Remedy—A review of all materials on site as well as expected to be onsite was performed and the inventory <br /> quantity has been updated into CERS for the location. <br /> HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content <br /> Remedy—The site map was re-written and includes all required content. This map has been uploaded into CERS. <br /> HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> Remedy—The Consolidated Emergency Response/Contingency Plan in CERS has been updated. The location <br /> Emergency Action Plan has been uploaded into CERS. <br /> HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Remedy—Training records were located and filed at the location, all location employees were re-trained. <br /> The CERS system has been updated and submitted for review.
