A site map shall contain a north orientation, loading areas, <br />internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation <br />staging areas, hazardous material handling and storage areas, and emergency response equipment. Immediately <br />log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or <br />updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />9 HSC 25508.2 Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br />The business plan information was not reviewed and resubmitted in the California Environmental Reporting System <br />(CERS) until March 16, 2017 . The hazardous materials inventory shall be submitted by January 15 of each <br />calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br />Employee training records were not available at the time of inspection. The business plan shall include provisions for <br />ensuring that appropriate personnel receive initial and annual refresher training. This training shall be documented <br />electronically or by hard copy and shall be made available for a minimum of three years. Immediately provide <br />employee training for appropriate personnel and submit a copy of the training records to the EHD. A site map was <br />not completed or submitted as part of the business plan. <br />This is a Class II violation. <br />12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br />Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br />response plans and procedures, the inventory of information required by this article, and the site map required by <br />this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br />for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br />plan readily available for review. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the completed inspection report. <br />Please be aware as of January 1, 2013 , all businesses were required to submit all hazardous materials information <br />online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
