The Facility information has not <br />been updated to reflect ownership change to CenturyLink. This information must be current and submitted as part <br />of the business plan. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />10 <br />HSC 25508.1(a) -(f) Failed to submit revisions to business plan within 30 days of substantial change in operations. <br />During the inspection, it was found that ownership has changed to CenturyLink as of November 2017 and the <br />updated information has not been submitted. Within 30 days of one the following events, business shall <br />electronically updatethe information submitted to the California Environmental Reporting System (CERS): <br />- A 100 percent or more increase in the quantity of a previously disclosed material. <br />- Any handling of a previously undisclosed hazardous materials <br />- A change of business address, business ownership, or business name. <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the completed inspection report. <br />Please be aware as of January 1, 2013, all businesses were required to submit all hazardous materials <br />information online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
