Box 6157 <br /> Stockton, CA 95206 <br /> Dear Business Owner: <br /> SUBJECT: PAYMENT OF CURRENT AND BACK PROGRAM FEES <br /> This letter serves as a formal notification that your Hazardous Materials Management Plan <br /> (HMMP) and Chemical Inventory has been received and reviewed by the San Joaquin County <br /> Office of Emergency Services (OES). However, our records indicate that your business has not <br /> been in compliance for prior years. The requirements of Chapter 6.95 of the California Health <br /> and Safety Code took effect in 1986. In these circumstances, County ordinance requires billing <br /> of back fees to 1990 or to the year that our records show your business exceeded program <br /> quantities. Back fees are calculated based on your current year fee calculations. <br /> If you have proof that your business was not in operation in San Joaquin County during the <br /> period of 1990 to the present, or that you did not handle hazardous materials in the specified <br /> quantities during that period, you can contact this office to have your back fees adjusted. <br /> Attached you will find a bill for current year and back fees. Please remit your payment to the <br /> Office of Emergency Services within 30 days of receipt of this letter. A 10% late fee will be <br /> assessed for delinquent payments. <br /> By law, all business licenses and building permits must be reviewed for compliance with the <br /> provisions of Chapter 6.95 of the California Health and Safety Code. If a business is delinquent <br /> in their annual fee, it could lead to delays or rejection of the license or permit process. <br /> Should you have any questions, please call OES at (209) 468-3969.
