The following is an itemized list of hazardous materials violations that have not been <br />addressed for EL CAMINO TIRES as of August24, 2018. <br />Open violations from September 30, 2016 inspection <br />Violation #3 - Failed to establish and implement a hazardous materials business plan. <br />A Hazardous Materials Business Plan has not been established or implemented. A Hazardous Materials Business <br />Plan shall be established and implemented for emergency response to a release or threatened release of a <br />hazardous material when storing hazardous materials at or above the thresholds quantities of 55 gallons for <br />materials that are liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperature and <br />pressure. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br />enter the required information, and submit for review by the EHD. <br />Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures have not been submitted. A business plan shall include the following <br />emergency response procedures for a release or threatened release of hazardous materials, including, but not <br />limited to, the following: <br />- immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br />- procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br />- evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />Violation #8 - Failed to submit a training program or program is not reasonable or adequate for facility. <br />A training program has not been submitted. Training programs for all employees and annual training, including <br />refresher courses, shall include safety procedures for the event of a release or threatened release of a hazardous <br />material, including, but not limited to, familiarity with the facility's emergency response plans and procedures. These <br />training programs may take into consideration of each employee. Immediately prepare or update the facility's <br />training program, log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br />upload the correct or updated information, and submit to the EHD for approval. <br />Violation #12 - Failed to have the business plan readily available to facility or EHD personnel. <br />Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br />response plans and procedures, the inventory of information required by this article, and the site map required by <br />this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br />for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br />plan readily available for review. <br />Open violations from September 04, 2015 inspection <br />Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures have not been submitted for the years 2013, 2014 and 2015. A business plan <br />shall include the following emergency response procedures for a release or threatened release of hazardous <br />materials, including, but not limited to, the following: <br />- immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br />- procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br />- evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />Page 1 of 2 <br />
