The following is an itemized list of hazardous materials violations that have not been <br /> addressed for ACCURATE TRANSMISSIONS as of October 15, 2015. <br /> Open violations from July 27, 2015 inspection <br /> Violation #1 -Failed to establish and implement a hazardous materials business plan. <br /> A Hazardous Materials Business Plan has not been established or implemented. A Hazardous Materials Business <br /> Plan shall be established and implemented for emergency response to a release or threatened release of a hazardot <br /> material when storing hazardous materials at or above the thresholds quantities of 55 gallons for materials that are <br /> liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperature and pressure. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> required information, and submit for review by the EHD. <br /> Violation#2 -Failed to electronically submit a business plan. <br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System <br /> (CERS). As of January 1, 2013, all businesses were required to submit all business plans electronically. Immediatel <br /> log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for <br /> approval. <br /> Violation#3 -Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar yea <br /> and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation#6 -Failed to complete and/or submit the Business Activities or Owner/Operator Identification <br /> page. <br /> The Business Owner/Operator Identification form was completed and submitted to CERS. This information must be <br /> current and submitted as part of the business plan. Immediately log into the California Environmental Reporting <br /> System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for <br /> approval. <br /> Violation#7 -Failed to complete and/or submit hazardous materials inventory for all reportable materials <br /> on site. <br /> Two 55 gallons drums of Oil (110 gallons total) were found on site that have not been reported. Any material that <br /> meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous <br /> material inventory, and submit to the EHD for approval. <br /> Violation#10 - Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed and submitted as part of the business plan. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment.
