NCOONTY <br />//C�s <br />RE: HAZMAT INVOICE/PERMIT REQUEST FOR VARIOUS LOCATIONS (SEE ATTACHED <br />REVISED LIST OF SITES) <br />Dear Robert: <br />This letter is to confirm my conversation with Michelle Halliwell on 3/7/12 when I called to inquire about <br />invoices for our AT&T Mobility cell sites. Per our conversation, Michelle stated that fees for unstaffed <br />sites are billed on the invoice for our primary site which is located at 6855 W. Eight Mile Road in <br />Stockton for which we have received an invoice. I also verified with Michelle that San Joaquin County <br />Office of Emergency Services does not issue permits for Hazardous Materials.
