The site map was not complete as part of the business plan. The site map is missing emergency shut offs(fuel pumps, propane tanks, and other chemical systems), utilities shut offs for each building (water, power, propane), hazardous material and handling areas, and exits for each building. The new site map with all of the required information will stay on site and be kept with the emergency dispatcher/operator on site because the site is classified as are the buildings floor plans. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a repeat violation, Class Il. 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened release. The emergency response procedures were not complete. The emergency response and contingency plan is missing the local Unified Program Agency(UPA)phone number. Please add 209-468-3420 as the local UPA phone number. Please add 925-447-6880 as your emergency response phone number for emergencies. Evacuation staging areas were not identified in the emergency response and contingency plan. Please identify your evacuation staging areas in the emergency response and contingency plan. A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to,the following: -immediate notification of local emergency personnel and the Environmental Health Department(EHD) -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment -evacuation plans and procedures, including immediate notice,for the site Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a Class II violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, within 30 days of receiving the inspection report. All photos were taken by a Lawrence Livermore National laboratory(LLNL)employee. All photos taken at LLNL have to go through a screening process before they can be released. A photo was taken by a LLNL employee of an emergency shut off button for the fuel pumps that was not identified on the site map.
