A site map <br />shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and <br />exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and <br />emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br />label as "NA". Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />% <br />HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures were lacking. Section H, data elements H2 and H4 have not been completed; <br />Section I, has not been updated to indicate the facility has no employees. A business plan shall include the <br />following emergency response procedures for a release or threatened release of hazardous materials, including, but <br />not limited to, the following: <br />- immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br />- procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br />- evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />9 <br />HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br />The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br />System (CERS) annually by January 15. The hazardous materials inventory shall be submitted by January 15 of <br />each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the <br />CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />An inspection report was provided to the facility operator on the day of inspection. The report was amended by <br />the EHD. This amended report replaces the initial report. Complete and submit a copy of the Return to <br />Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or <br />will be taken for each violation, and any supporting paperwork, within 30 days of receiving the completed <br />inspection report. <br />Violation 9 was amended to remove "corrected on site" and violation 12 was removed from the checklist. <br />Please be aware as of January 1, 2013, all businesses were required to submit all hazardous materials <br />information online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
