The facility's business plan, with all of the required information,was not submitted electronically to the California Environmental Reporting System(CERS) for the years 2013, 2014 and 2015. As of January 1, 2013, all businesses were required to submit all business plans electronically. Immediately log into the CERS at hftp://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 3 HSC 25508.2 Failed to annually review and certify that the business plan is complete, accurate, and up to date. The business plan information has not been completed, reviewed, certified and resubmitted in the California Environmental Reporting System(CERS) annually for the years 2013, 2014 and 2015. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 6 HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. The Business Owner/Operator Identification form was not submitted for the years 2013 and 2014 and the 2015 submittal was not complete. The Secondary Emergency Contact can not be the same person as the Primary Emergency Contact and title of the signer/document preparer has not been identified. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at http:llcers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a Class II violation. 7 HSC 25506(a) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. Carbon dioxide stored in a 400 pound bulk storage container, four 50 pound and two 20 pound cylinders were found on site that have not been reported for the years 2013, 2014 and 2015. Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan.
