The Secondary Emergency Contact is <br /> identified as Allen Birks who has not been employed by Bulk Transportation for at least six months. This <br /> information must be current and submitted as part of the business plan. Immediately log into the California <br /> Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/, enter the correct or updated information, and <br /> submit to the EHD for approval. <br /> This is a Class II violation. <br /> 5 HSC 25506(a) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> A 55 gallon drum of waste antifreeze was found on site that has not been reported. Any material that meets or <br /> exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California <br /> Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the materials to the hazardous material <br /> inventory, and submit to the EHD for approval. <br /> The Hazardous Materials Inventory forms for used oil, new engine oil, spent oily debris, waste engine oil filters, <br /> antifreeze&Coolant, pyroplex red N2Gi#2 and cerulean grease wrongly identifies the location of these materials as <br /> the maintenance shop vs. the hazardous materials storage shed, outside and south of the maintenance shop;the <br /> Hazardous Materials Inventory form for new engine oil identifies the maximum amount as 110 gallon vs. 550 <br /> gallons;the Hazardous Materials Inventory form for waste engine oil filters wrongly shows the amount of filter in <br /> gallons vs. pounds. This information must be current and submitted as part of the business plan. Immediately log <br /> into the California Environmental Reporting System(CERS) at http:/Icers.calepa.ca.gov/, enter the correct or <br /> updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The site map for the maintenance shop does not show the storage location for drum of waste antifreeze, one of two <br /> eye wash stations, electrical shutoff, bay door on the north side of the facility and the actual location of the spill kit <br /> and fire extinguisher inside the hazardous materials storage shed.
