The following is an itemized list of hazardous materials violations that have not been <br />addressed for Electrosteel USA as of June27, 2016. <br />Open violations from May 04, 2016 inspection <br />Violation #2 - Failed to electronically submit a business plan. <br />The facility's business plan was not submitted electronically to the California Environmental Reporting System <br />(CERS). As of January 1, 2013, all businesses were required to submit all business plans electronically. Immediatel <br />log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for <br />approval. <br />Violation #3 - Failed to establish and implement a hazardous materials business plan. <br />A Hazardous Materials Business Plan has not been established or implemented. A Hazardous Materials Business <br />Plan shall be established and implemented for emergency response to a release or threatened release of a hazardot <br />material when storing hazardous materials at or above the thresholds quantities of 55 gallons for materials that are <br />liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperature and pressure. <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br />required information, and submit for review by the EHD. <br />Violation #4 - Failed to complete and/or submit the Business Activities or Owner/Operator Identification <br />page. <br />The Business Activities and the Business Owner/Operator Identification forms have not been submitted. This <br />information must be current and submitted as part of the business plan. Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and <br />submit to the EHD for approval. <br />Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials <br />on site. <br />Two 55 gallon drums of diesel and three 55 gallon drums of flammable paint were found on site that have not been <br />reported. Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the <br />materials to the hazardous material inventory, and submit to the EHD for approval. <br />Violation #6 - Failed to complete and/or submit a site map with all required content. <br />A site map has not been submitted. A site map shall contain a north orientation, loading areas, internal roads, <br />adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, <br />hazardous material handling and storage areas, and emergency response equipment. Immediately log into the <br />California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated <br />information, and submit to the EHD for approval. <br />Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures have not been submtted. A business plan shall include the following emerges, <br />response procedures for a release or threatened release of hazardous materials, including, but not limited to, the <br />following: <br />- immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br />- procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br />- evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />Page 1 of 2 <br />
