The following is an itemized list of hazardous materials violations that have not been <br /> addressed for DEL RIO WEST PALLET as of December31, 2015. <br /> Open violations from September 28,2015 inspection <br /> Violation#2 - Failed to electronically submit a business plan. <br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System (CER: <br /> for 2015. As of January 1, 2013, all businesses were required to submit all business plans electronically. <br /> Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to <br /> the EHD for approval. <br /> Violation#3 -Failed to annually review and certify that the business plan is complete, accurate,and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)for 2015. The hazardous materials inventory shall be submitted by January 15 of each calendar yea <br /> and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation #6 - Failed to complete and/or submit the Business Activities or Owner/Operator Identification <br /> page. <br /> The Business Owner/Operator Identification form was not submitted for 2015. This information must be current and <br /> submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) <br /> at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> The Business Owner/Operator Identification form was not updated with the EPA ID Number and the Secondary <br /> Emergency has not been updated as required. This information must be current and submitted as part of the <br /> business plan. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation #7 - Failed to complete and/or submit hazardous materials inventory for all reportable materials <br /> on site. <br /> Reportable quantities of propane, diesel, oxygen and waste oil were found on site that have not been reported for 20 <br /> Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the <br /> materials to the hazardous material inventory, and submit to the EHD for approval. <br /> Violation#10 -Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed or submitted as part of the business plan for 2015. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment. Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> Violation #11 -Failed to establish and/or submit emergency response procedures for a release or <br /> threatened release. <br /> The emergency response procedures have not been submitted for the years 2013, 2014 and 2015. A business plan <br /> shall include the following emergency response procedures for a release or threatened release of hazardous materia <br /> including, but not limited to, the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Page 1 of 2 <br />
