The following is an itemized list of hazardous materials violations that have not been addressed for DEL RIO WEST PALLET as of December 10, 2015. Open violations from September 28, 2015 inspection Violation#2- Failed to electronically submit a business plan. The facility's business plan was not submitted electronically to the California Environmental Reporting System (CER: for 2015. As of January 1, 2013, all businesses were required to submit all business plans electronically. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. Violation#3 - Failed to annually review and certify that the business plan is complete,accurate, and up to date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System (CERS)for 2015. The hazardous materials inventory shall be submitted by January 15 of each calendar yea and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. Violation #6 -Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. The Business Owner/Operator Identification form was not submitted for 2015. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. The Business Owner/Operator Identification form was not updated with the EPA ID Number and the Secondary Emergency has not been updated as required. This information must be current and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. Violation #7 -Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. Reportable quantities of propane, diesel, oxygen and waste oil were found on site that have not been reported for 20 Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. Violation#10 - Failed to complete and/or submit a site map with all required content. A site map was not completed or submitted as part of the business plan for 2015. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. Violation#11 - Failed to establish and/or submit emergency response procedures for a release or threatened release. The emergency response procedures have not been submitted for the years 2013, 2014 and 2015. A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materia including, but not limited to; the following: -immediate notification of local emergency personnel and the Environmental Health Department(EHD) - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment -evacuation plans and procedures, including immediate notice, for the site Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval.
