The following is an itemized list of hazardous materials violations that have not been <br /> addressed for EL DORADO ST PARTNERS,LP GO CBRE as of October 04, 2018. <br /> Open violations from August 01, 2018 inspection <br /> Violation#4- Failed to complete and/or submit the Business Activities or Owner/Operator Identification <br /> page. <br /> The Business Owner/Operator Identification form was not submitted or accurate. Dee Dee Hinze does not work for <br /> EI Dorado St Partners, LP and is not the Environmental Contact. This information must be current and submitted <br /> as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation#6 - Failed to complete and/or submit a site map with all required content. <br /> The maps do not show loading areas, storm and sewer drains, access and exit points, emergency shutoffs, <br /> evacuation staging areas, hazardous material handling and storage area (diesel fuel), emergency response <br /> equipment(fire extinguishers, spill control, first aid), and did not include a legend to identify the <br /> acronyms/abbreviations used on the map. A site map shall contain a north orientation, loading areas, internal <br /> roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging <br /> areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element <br /> is not applicable for your facility then list it on the map and label as "NA". Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, <br /> and submit to the EHD for approval. <br /> Violation#7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were lacking. Section C, the phone numbers for the Local Unified Program <br /> Agency, nearest medical facility, and the Regional Water Quality Control Board have not been identified; Section E, <br /> the evacuation assembly areas have not been identified; Section G, the location of each emergency response <br /> equipment checked has not been identified; Section H has not been completed; Section K has not been signed. A <br /> business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Training records Denise Daniel or any other employee at this facility were not available at the time of inspection. <br /> The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual <br /> refresher training. This training shall be documented electronically or by hard copy and shall be made available for <br /> a minimum of three years. Immediately provide employee training for appropriate personnel and submit a copy of <br /> the training records to the EHD. <br /> Violation #12 - Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
