The site map was missing north orientation, loading <br /> areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs(utilities <br /> shut offs), evacuation staging area, hazardous material handling and storage areas, and emergency response <br /> equipment(fire extinguisher, spill kit, eye wash and first aid it). <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into <br /> the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually. The business did not make a submittal between 11/15/2017- 1/15/2018.The last <br /> submittal was made on 1/16/2018 making it late. This violation was corrected on site.The hazardous materials <br /> inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of <br /> the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 1/17/2019. This inspection was supposed to be a closing inspection. The diesel in the generator <br /> and the propane on site is keeping this business from being able to be inactivated from the Hazardous Material <br /> Business Plan program. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
