Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />August 20, 2018 <br /> Facility Address: <br /> 14000 E HWY 88 , LOCKEFORD <br /> Facility Name: <br /> COLONIAL ENERGY CE 40138 (DBA POWER <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />A site map is missing KEY FOR LPG, PPE, MSDS. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br />access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br />areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br />map and label as β€œNA". Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures field C3 has the wrong CUPA number listed, change to 209-468-3420. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br />hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br /> 12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br />Facility could not provide the Emergency Response Plan upon request. The plan printed at the facility did not have <br />the Emergency Response plan. <br /> The emergency response plans and procedures, the inventory of information required by this article, and the site <br />map required by this section shall be readily available to personnel of the business or the unified program facility <br />with responsibilities for emergency response or training pursuant to this section. Immediately make arrangements <br />to have the business plan readily available for review. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 9/19/2018. <br />Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials <br />information online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
