BOX 6039 <br /> STOCKTON, CA 95206-0039 <br /> SUBJECT: CARBON DIOXIDE SURVEY FORM REGARDING 1304 HAMMER LANE, <br /> SUITE#3 IN STOCKTON, CA 95210 <br /> Chapter 6.95 of the California State Health and Safety Code requires businesses handling <br /> hazardous materials to prepare Hazardous Materials Management Plans (HMMPs) for their <br /> employees' use in an emergency. Businesses must also submit copies of these plans, along with <br /> an annual inventory, to public safety agencies for their use in protecting responders and the public. <br /> The inventory information is also made available to the public under community right-to-know <br /> provisions of the law. <br /> We are currently surveying the food industry operating in San Joaquin County to determine which <br /> facilities are using a regulated hazardous material, carbon dioxide, in their operations. The <br /> enclosed survey form will allow you to meet this reporting requirement. Please complete and <br /> return this form after reading the information on its reverse side. If you own or operate more than <br /> one business within San Joaquin County, please complete a survey form for each location. Your <br /> response is required by State law.
