The following is an itemized list of hazardous materials violations that have not been <br /> addressed for HEADWATERS RESOURCES INC as of May03, 2016. <br /> Open violations from January 26, 2016 inspection <br /> Violation #3 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar yee <br /> and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation #6 -Failed to complete and/or submit the Business Activities or Owner/Operator Identification <br /> page. <br /> The Business Owner/Operator Identification Form and the Business Activities form were not submitted for the years <br /> 2013, 2014, 2015 and 2016. This information must be current and submitted as part of the business plan. <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation #11 - Failed to establish and/or submit emergency response procedures for a release or <br /> threatened release. <br /> The emergency response procedures have not been submitted for the years 2013, 2014, 2015 and 2016. A busines <br /> plan shall include the following emergency response procedures for a release or threatened release of hazardous <br /> materials, including, but not limited to, the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Page 1 of 1 <br />
