The following is an itemized list of hazardous materials violations that have not been <br />addressed for SIERRA CHEMICAL CO (STKN) as of December 31, 2015. <br />Open violations from October 30, 2015 inspection <br />Violation #7 - Failed to complete and/or submit hazardous materials inventory for all reportable materials <br />on site. <br />Reportable quantities of soda ash, sodium bicarbonate, calcium chloride, aluminum sulfate, polyaluminum chloride <br />solution, citric acid solution, sodium chloride, citric acid solution, oxalic acid dihydrate and latex paint were found on <br />site that have not been reported. Any material that meets or exceeds the reportable quantity shall be reported in the <br />facility's business plan. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval <br />The Hazardous Materials Inventory forms for calcium hypochlorite, sodium bisulfite, muriatic acid were not complete. <br />The amount for each of these materials are identified in pounds instead of gallons . This information must be currer <br />and submitted as part of the business plan. Immediately log into the California Environmental Reporting System <br />(CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />Violation #10 - Failed to complete and/or submit a site map with all required content. <br />A site maps are not complete. North orientation, spill control equipment, hazmat storage locations for the oxygen, <br />acetylene and latex paint are not shown. A site map shall contain a north orientation, loading areas, internal roads, <br />adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, all <br />hazardous material handling and storage areas, and emergency response equipment. Immediately log into the <br />California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated <br />information, and submit to the EHD for approval. <br />Violation #13 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br />Proof of training was not available at the time of inspection. The business plan shall include provisions for ensuring <br />that appropriate personnel receive initial and annual refresher training. This training shall be documented electronic <br />or by hard copy and shall be made available for a minimum of three years.
