The following is an itemized list of hazardous materials violations that have not been addressed for SIERRA CHEMICAL CO (STKN) as of December 31, 2015. Open violations from October 30, 2015 inspection Violation #7 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. Reportable quantities of soda ash, sodium bicarbonate, calcium chloride, aluminum sulfate, polyaluminum chloride solution, citric acid solution, sodium chloride, citric acid solution, oxalic acid dihydrate and latex paint were found on site that have not been reported. Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval The Hazardous Materials Inventory forms for calcium hypochlorite, sodium bisulfite, muriatic acid were not complete. The amount for each of these materials are identified in pounds instead of gallons . This information must be currer and submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. Violation #10 - Failed to complete and/or submit a site map with all required content. A site maps are not complete. North orientation, spill control equipment, hazmat storage locations for the oxygen, acetylene and latex paint are not shown. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, all hazardous material handling and storage areas, and emergency response equipment. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. Violation #13 - Failed to provide or document initial and/or refresher training to appropriate personnel. Proof of training was not available at the time of inspection. The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be documented electronic or by hard copy and shall be made available for a minimum of three years.
