Environmental Health Department: <br />Gary Cummings was unable to provide any consolidated manifests for the past <br />three years. Records of all hazardous waste disposal shall be kept for 3 years. <br />This is in violation of HSC 25160.2. <br />Sierra Chemical Response and Action: <br />We did not fail to keep signed copies of consolidated manifests for three years. <br />No hazardous waste material had been shipped under a consolidated manifest <br />during that time. Invoices were provided to demonstrate that waste oil was <br />removed from the facility by the contractor maintaining our forklifts. Our truck <br />fleet was serviced offsite. <br />Cost: Cost to collect documents and respond. $ 500.00 <br />Complaint: <br />j. Failed to retain a copy of manifest or bill of lading for spent lead acid batteries for <br />three (3) years, in violation of California Code of Regulations, Title 22, section <br />66266.81(a)(4)(B). <br />Environmental Health Department: <br />No records of lead acid battery disposal were available for reviews. Records of <br />all hazardous waste disposal shall be kept for 3 years. This is a violation of CCR <br />66266.81(a)(4)(B). <br />Sierra Chemical Response and Action: <br />Copies of the invoices relating to lead acid batteries being returned for core were <br />not readily available at our Stockton facility. They were transmitted from our <br />Sparks Corporate office by letter dated August 30, 2005. <br />Cost: Cost of training. $ 500.00 <br />Sierra Chemical Response and Action: <br />Complaint: <br />k. Failed to maintain completed personnel training records, in violation of California <br />Code of Regulations, Title 22, section 66265.16. <br />Environmental Health Department: <br />Sierra Chemical was unable to provide personnel training records for the current <br />year. This is in violation of CCR, Title 22, Section 66265.16(b)(c)(d)&(e). <br />Sierra Chemical Response and Action: <br />Training was provided by Jeff Kinder of CFR Services on August 22, 2005. <br />Copies of the signoff sheet were provided to Ms Foley by transmittal letter dated <br />August 30, 2005. <br />Cost: Cost of training $ 1,000.00 <br />
