The following is an itemized list of hazardous materials violations that have not been <br /> addressed for SIERRA CHEMICAL CO(STKN) as of May 31, 2016. <br /> Open violations from October 30, 2015 inspection <br /> Violation#7-Failed to complete and/or submit hazardous materials inventory for all reportable materials <br /> on site. <br /> Reportable quantities of soda ash, sodium bicarbonate, calcium chloride, aluminum sulfate, polyaluminum chloride <br /> solution, citric acid solution, sodium chloride, citric acid solution, oxalic acid dihydrate and latex paint were found on <br /> site that have not been reported. Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. <br /> The Hazardous Materials Inventory forms for calcium hypochlorite, sodium bisulfite, muriatic acid were not complete. <br /> The amount for each of these materials are identified in pounds instead of gallons . This information must be currer <br /> and submitted as part of the business plan. Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation#10 - Failed to complete and/or submit a site map with all required content. <br /> A site maps are not complete. North orientation, spill control equipment, hazmat storage locations for the oxygen, <br /> acetylene and latex paint are not shown. A site map shall contain a north orientation, loading areas, internal roads, <br /> adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, all <br /> hazardous material handling and storage areas, and emergency response equipment. Immediately log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated <br /> information, and submit to the EHD for approval. <br /> Violation#13 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Proof of training was not available at the time of inspection. The business plan shall include provisions for ensuring <br /> that appropriate personnel receive initial and annual refresher training. This training shall be documented electronics <br /> or by hard copy and shall be made available for a minimum of three years. Immediately provide employee training fc <br /> appropriate personnel and submit a copy of the training records to the EHD. <br /> Page 1 of 1 <br />
