AN Environmental Health Department <br /> QU <br /> COUNTY— <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> GRUPE COMMERCIAL COMPANY HANGER#2 1 5000 S LINDBERGH ST, STOCKTON September 20, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> The Hazardous Materials Inventory form for jet fuel was not accurate. The storage temperature has wrongly been <br /> identified as below ambient vs. ambient. This information must be current and submitted as part of the business <br /> plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The map did not show the fuel loading area, storm and sewer drains, emergency shutoffs, emergency response <br /> equipment (fire extinguisher, spill control, first aid, and South Lindbergh St. (the address of the facility is identified as <br /> 5000 S Lindbergh St). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, <br /> storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material <br /> handling and storage areas, and emergency response equipment. If a site map element is not applicable for your <br /> facility then list it on the map and label as "NA". Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were lacking. Section C. the phone numbers for the Local Unified Program <br /> Agency,209-468-3420 and the phone number for the Regional Water Quality Control Board, 916-464-3291 have <br /> not been identified and the name and phone number of the nearest medical facility has not been identified; Section <br /> E. the Emergency Assembly Area has not been identified; Section G. the Safety and First Aid and Communications <br /> and Alarm Systems have not been completed; Section H. has not been completed. A business plan shall include <br /> the following emergency response procedures for a release or threatened release of hazardous materials, including, <br /> but not limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year.
