The following is an itemized list of hazardous materials violations that have not been <br /> addressed for Reach 02 as of December 12, 2018. <br /> Open violations from August 31, 2018 inspection <br /> Violation#6 -Failed to complete and/or submit a site map with all required content. <br /> The map(s) did not show a north orientation (except for the evacuation meeting area map), loading areas, internal <br /> roads, storm and sewer drains, emergency shutoffs, emergency response equipment, and the symbol used to <br /> denote the primary and secondary meeting areas are not included in the legend. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br /> "NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> Violation#7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were lacking. Section C. the phone numbers for the Local Unified Program <br /> Agency, 209-468-3420 and the phone number for the Regional Water Quality Control Board, 916-464-3291 have <br /> not been identified. A business plan shall include the following emergency response procedures for a release or <br /> threatened release of hazardous materials, including, but not limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation#12 - Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
