The local Unified Program Agency (UPA) phone number <br />listed is not correct on line C3 in the consolidated emergency response and contingency plan.. Please list <br />209-468-3420 as the local UPA phone number on line C3. <br />A business plan shall include the following emergency response procedures for a release or threatened release of <br />hazardous materials, including, but not limited to, the following: <br />- immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br />- procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br />- evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br />The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br />System (CERS) annually. The sunmital was not made between 11/1/2017 -1/15/2018. The last complete submittal <br />was made on 5/8/2018 making the submittal late. <br />The hazardous materials inventory shall be submitt <br />ed by January 15 of each calendar year and may be submitted <br />beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Cert <br />ification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 12/16/2018 . <br />Start <br />ing September 1, 2018, all in -office CERS help will be provided at EHD hourly rate ($152).
