Dennis Noss has not worked for <br />the facility for at least a year and five months. This information must be current and submitted as part of the business <br />plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter <br />the correct or updated information, and submit to the EHD for approval. <br />This was corrected on site. <br />This is a Class II violation. <br />9 <br />HSC 25508.2 Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br />The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br />System (CERS) annually for 2016 and 2017 The hazardous materials inventory shall be submitted by January 15 of <br />each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the <br />CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This was corrected on site. <br />This is a Class II violation. <br />11 <br />HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br />Employee training records were not available at the time of inspection. The business plan shall include provisions for <br />ensuring that appropriate personnel receive initial and annual refresher training. This training shall be documented <br />electronically or by hard copy and shall be made available for a minimum of three years. Immediately provide <br />employee training for appropriate personnel and submit a copy of the training records to the EHD. A site map was <br />not completed or submitted as part of the business plan. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the completed inspection report. <br />Assisted Mr. Moss with CERS account setup, access and submittal. <br />Please be aware as of January 1, 2013, all businesses were required to submit all hazardous materials informatio <br />online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
