The following is an itemized list of hazardous materials violations that have not been <br /> addressed for OLD SPAGHETTI FACTORY, THE as of March 12, 2018. <br /> Open violations from January 08, 2018 inspection <br /> Violation#6 -Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed or submitted as part of the business plan. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br /> "NA". Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> Need loading area, storm/sewer areas, access/exit points, emergency shut offs, evacuation stagging areas, <br /> hazardous materials area, emergency equipment <br /> Violation#7-Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were lacking or inadequate. A <br /> business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation #8 - Failed to submit a training program or program is not reasonable or adequate for facility. <br /> A training program was lacking or insufficient. [Load information to CERS] . Training programs for all employees <br /> and annual training, including refresher courses, shall include safety procedures for the event of a release or <br /> threatened release of a hazardous material, including, but not limited to, familiarity with the facility's emergency <br /> response plans and procedures. These training programs may take into consideration of each employee. <br /> Immediately prepare or update the facility's training program, log into the California Environmental Reporting <br /> System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for <br /> approval. <br /> Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation#11 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> [Last training not dated.] . The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training. This training shall be documented electronically or by hard copy and <br /> shall be made available for a minimum of three years.
