The following is an itemized list of hazardous materials violations that have not been <br /> addressed for Terrill Transportation as of December10, 2018. <br /> Open violations from October 10, 2018 inspection <br /> Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> The Hazardous Materials Inventory forms for the entire inventory are not complete. The size of the largest container <br /> and days on site have not been completed for each item reported; the annual waste amount and state waste codes <br /> have not been identified for the used/waste material; the type of material for the used ethylene Glycol is wrongly <br /> identified as "pure" vs. "waste"; the type of material for the used waste motor oil is wrongly identified as"mixture" vs. <br /> "waste", the storage pressure and temperature have not been completed, and the name does not contain the word <br /> "waste"; the storage pressure and temperature for acetylene has not been completed; the storage pressure for <br /> carbon dioxide and oxygen has been identified at"ambient" vs. "above ambient'; This information must be current <br /> and submitted as part of the business plan. Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> The map does not contain a north orientation, adjacent streets, storm and sewer drains, access and exit points <br /> (from the street), evacuation staging areas, and emergency response equipment(fire extinguishers and spill <br /> control). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and <br /> sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling <br /> and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then <br /> list it on the map and label as"NA". Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were lacking. Section C. the nearest medical facility/hospital name and <br /> phone number have not been identified; the phone number for the Regional Water Quality Control Board, <br /> 916-464-3291 has not been identified; Section G. Location of the telephones have not been completed. A business <br /> plan shall include the following emergency response procedures for a release or threatened release of hazardous <br /> materials, including, but not limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation#11 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> Violation#12 - Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
