Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The map does not contain a north orientation, adjacent streets, storm and sewer drains, access and exit points <br /> (from the street), evacuation staging areas, and emergency response equipment (fire extinguishers and spill <br /> control). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and <br /> sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling <br /> and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then <br /> list it on the map and label as "NA". Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were lacking. Section C. the nearest medical facility/hospital name and <br /> phone number have not been identified; the phone number for the Regional Water Quality Control Board, <br /> 916-464-3291 has not been identified; Section G. Location of the telephones have not been completed.
