As of January 1, 2013, all businesses were required to submit all business plans electronically, This <br />violation was corrected on site, <br />This Is a Class II violation. <br />3 <br />HSC 25507 Failed to establish and implement a hazardous materials business plan. <br />A Hazardous Materials Business Plan has not been established or Implemented. A Hazardous Materials Business <br />Plan shall be established and Implemented for emergency response to a release or threatened release of a <br />hazardous material when storing hazardous materials at or above the thresholds quantities of 55 gallons for <br />materials that are Ilqulds, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperature and <br />pressure, This violation has been corrected on site. <br />This is a Class II violation. <br />4 <br />HSC 25508(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identlflcatlon page. <br />The Business Owner/Operator Identification form was not submitted , This Information must be current and <br />submitted as part of the business plan. This violation has been corrected on site, <br />This Is a minor violation. <br />S <br />HSC 25506(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br />1,200 gallons of flammable paint, 225 gallons of diesel, 7,800 cubic feet of acetylene, 7,800 cubic fet of oxygen, <br />3,000 pounds of carbon dioxide, 3,000 gallons of argon, 350 gallons of new oil, 250 gallons of used 011, 305 gallons <br />of acetone, and 250 gallons of paint waste were found on site that have not been reported. Any material that meets <br />or exceeds the reportable quantity shall be reported In the facility's business plan. <br />This violation was corrected on site. <br />This is a minor violation. <br />MSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />6 <br />A site map was not completed or submitted. A site map shall contain a north orientation, loading areas, internal <br />roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging <br />areas, hazardous material handling and storage areas, and emergency response equipment.
