<br />HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site.
<br />Thirty-six 5 gallon pails of cold pour crack filler, one 55 gallon drum of deSent 0119, thirty-seven 275 gallon totes of
<br />Butonal NX 4190, five 55 gallon drums of waste oil, one 55 gallon drum of waste antifreeze, and one 55 gallon drum
<br />of BTW 5461 were found on site that have not been reported. Any material that meets or exceeds the reportable
<br />quantity shall be reported in the facility's business plan. Immediately log into the California Environmental Reporting
<br />System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to
<br />the EHD for approval.
<br />The Hazardous Materials Inventory forms were incomplete. The "hazardous material type" for multi-purpose boiler
<br />treatment, drill chill, oxygen 02, liquid asphalt, muriatic acid, AA -75, emulsifier E7000, latex mixture, SS -1 H anionic
<br />emulsion, CRS -2H cationic emulsion, PMCRS-2H, asphalt flux, biosolvent, asphalt release agent, motor,
<br />transmission and, hydraulic, degreaser, emulsifier E4819, warm mix additive, polyethylene wax and synthetic wax,
<br />alkyl sulfonic acids, SBS polymer, thermoplastic copolymer, LP -Gas, sealcoat has not been completed. This
<br />information must be current and submitted as part of the business plan. Immediately log into the California
<br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and
<br />submit to the EHD for approval.
<br />This is a Class II violation.

<br />HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content.
<br />The map(s) did not show storm and sewer drains, access and exit points (site and structures), emergency response
<br />equipment (fire extinguishers, first aid, and spill control), and the hazardous material storage in the old shop, outside
<br />west wall of the truck shop, the 40 foot container south of the truck shop, the Tote storage outside of the tank farm
<br />area, the corrosive storage in the boiler area, and the hazardous materials stored inside and outside of warehouse
<br />1; some codes/symbols used on the map were not included in the legend; some parts of the map are not legible. A
<br />site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains,
<br />access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage
<br />areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the
<br />map and label as "NA". Immediately log into the California Environmental Reporting System (CERS) at
<br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval.
<br />This is a Class II violation.
<br />Overall Inspection Comments:
<br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement
<br />documenting the corrective actions that have been or will be taken for each violation, and any supporting
<br />paperwork, within 30 days of receiving the completed inspection report.
<br />Facility does not have internal roads and this element was not addressed on the site map. Please address this
<br />by either writing "NA" for not applicable or a statement that says the element is not on site. Please correct this
<br />by the next submittal.
