BOX 55242 <br />STOCKTON CA 95205 <br />SUBJECT: HAZARD MATERIALS DISCLOSURE SURVEY FOR: <br />3550 S HWY 99, STOCKTON 95215 <br />Division 6.95 of the California State Health and Safety Code requires businesses handling <br />hazardous materials to prepare Hazardous Materials Management Plans (HMMPs) for their <br />employee's use in an emergency. Businesses must also submit copies of these plans, along with <br />an annual inventory, to public safety agencies for their use in protecting responders and the <br />public. The inventory information is also made available to the public under right -to -know <br />provisions of the law. <br />Businesses operating in San Joaquin County must disclose to our office whether they are using <br />hazardous materials in their operations. The attached survey form will allow you to meet this <br />requirement. Please complete and return this form after reading the information on its reverse <br />side. Your response is required by State law.
