EMERGENCY ACTION & NOTIFICATION LIST <br />TO BE KEPT WITH TRUCK DOCUMENTS & DRIVERS FOLDER <br />IN CASE OF EMERGENCY EMPLOYEE IS TO: <br />1. If safe to do so, stop product flow (close manifold valve, shut down pump, etc.) <br />2. Call 911 if any injuries occur. <br />3. If necessary, quicklycontain or divert further contamination or contain product spill. <br />4. IMMEDIATELY Notify qualified individuals listed below and stand by until a qualified individual <br />arrives or as you are otherwise instructed. <br />QUALIFIED INDIVIDUAL RESPONSIBILITIES: <br />1. The following individuals are qualified to respond on site: Mike Grogan, Mike Keys or Richard <br />Erickson, <br />2. Perform agency notifications (as required). <br />3. Activate clean up procedures. Notify spill contractor(s) if necessary. <br />4. Call water treatment plant (if spill enters storm drain). <br />S.
