The following is an itemized list of hazardous materials violations that have not been <br /> addressed for O'Reilly Auto Parts DC #23 as of December 07, 2018. <br /> Open violations from October 23, 2018 inspection <br /> Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> 560 cubic feet of compressed oxygen, 220 cubic feet of acetylene, 185 gallon tank of used oil, 55 gallons of simple <br /> green, at least eight 55 gallon drums of antifreeze, two 55 gallon drums of super clean, two hundred twenty-seven <br /> 30 pound cylinders of Freon 134a, and at least 55 gallons of diesel exhaust were found on site that have not been <br /> reported. Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the <br /> materials to the hazardous material inventory, and submit to the EHD for approval. <br /> The Hazardous Materials Inventory forms for motor oil, waste flammable liquids, combustible liquids, corrosive <br /> liquids, flammable gas, flammable liquids, oxidizing liquids, and water reactive liquids. The amount of motor oil is <br /> thousands of gallons more than the 110 gallons reported; the amount waste liquids were shown in pounds vs <br /> gallons; the chemical name for the combustible liquids, corrosive liquids, flammable gas, flammable liquids, <br /> oxidizing liquids, and water reactive liquids have not been identified. This information must be current and <br /> submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) <br /> at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation#6 - Failed to complete and/or submit a site map with all required content. <br /> The map does not show a loading areas, access and exit points, emergency shutoffs, hazardous material handling <br /> and storage areas for the compressed oxygen, acetylene, used oil, simple green, antifreeze, super clean, Freon <br /> 134a, diesel exhaust fluid, diesel exhaust fluid, corrosive liquids, flammable gas, flammable liquids, oxidizing liquids, <br /> water reactive liquids, bulk oil storage area, battery room and the batteries stored at the south east corner of the <br /> warehouse. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and <br /> sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling <br /> and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then <br /> list it on the map and label as "NA". Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> Violation #10 - Failed to submit revisions to business plan within 30 days of substantial change in <br /> operations. <br /> During the inspection, it was found that 560 cubic feet of compressed oxygen, 220 cubic feet of acetylene, 185 <br /> gallon tank of used oil, 55 gallons of simple green, at least eight 55 gallon drums of antifreeze, two 55 gallon drums <br /> of super clean, two hundred twenty-seven 30 pound cylinders of Freon 134a, at least 55 gallons of diesel exhaust <br /> fluid and thousands of gallons of motor oil were found on site and the updated information has not been submitted. <br /> Within 30 days of one the following events, business shall electronically update the information submitted to the <br /> California Environmental Reporting System (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> Page 1 of 1 <br />
