Hazardous Materials Program Inspection Report <br />Date: <br />February 28, 2017 <br /> Facility Address: <br /> 109 N SCHOOL ST, LODI <br /> Facility Name: <br /> LODI STADIUM 12 <br />San Joaquin County <br />Environmental Health Department <br />1868 East Hazelton Avenue, Stockton, California 95205-6232 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sjgov.org/ehd <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 2 HSC 25505, 25508(a)(1) Failed to complete and electronically submit a hazardous materials business plan. <br />The facility's business plan was not submitted electronically to the California Environmental Reporting System <br />(CERS). As of January 1, 2013, all businesses were required to submit all business plans electronically. <br />Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to <br />the EHD for approval. <br />This is a minor violation. <br /> 5 HSC 25506(a) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br />The Hazardous Materials Inventory forms for one 400 pound cylinder of CO2, one 35 pound cylinder of CO2, and <br />three 50 pound cylinders of CO2 were not complete. This information must be current and submitted as part of the <br />business plan. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />A site map was not completed as part of the business plan. A site map shall contain a north orientation, loading <br />areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, <br />evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br />release. <br />The emergency response procedures inadequate. Phone number for regional water quality control board lacking. <br />Emergency response procedures were not available during inspection. A business plan shall include the following <br />emergency response procedures for a release or threatened release of hazardous materials, including, but not <br />limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department (EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br />persons, property, or the environment <br /> - evacuation plans and procedures, including immediate notice, for the site <br />Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br />correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br />Page 2 of 3
