A site map shall contain a north <br />orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency <br />shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. <br />If a site map element is not applicable for your facility then list it on the map and label as “NA". Immediately log into the <br />California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated <br />information, and submit to the EHD for approval. <br />This is a minor violation. <br />#6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content <br /> #9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to <br />date <br />OBSERVATIONS: The business plan information has not been reviewed and resubmitted in the California Environmental <br />Reporting System (CERS) annually. The hazardous material business plan is late because it was submitted after <br />1/15/2018. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be <br />submitted beginning November 1 of the previous year. This violation has been corrected on site. <br />This is a Class II violation. <br />#9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to <br />date <br />OVERALL NOTES AND COMMENTS <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the <br />corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 8/10/2018. <br />Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials information online to <br />the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
