A letter was sent out to all facilities who are past due recently, <br />regarding this annual requirement (by Jan. 15), to resubmit the information in CERS. Please contact me at your earliest <br />opportunity in order to log onto CERS and make the submittal. You can also schedule an appointment to come into the <br />office and complete the submittal on Wednesdays by calling our main phone number at (209) 468-3420.
