The following is an itemized list of hazardous materials violations that have not been <br /> addressed for CHIPOTLE MEXICAN GRILL #1556 as of May 11, 2016. <br /> Open violations from November 04, 2015 inspection <br /> Violation #8 - Failed to submit revisions to HMBP within 30 days of change in business info or inventory. <br /> During the inspection, it was found that the primary emergency contact is no longer at this facility and the updated <br /> information has not been submitted. Within 30 days of one the following events, business shall electronically update <br /> the information submitted to the California Environmental Reporting System (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation #1010 - Unlisted Administration/Documentation violation. <br /> Hazardous Materials Business Plan was not available for inspection. There was no hard copy of the HMBP and the <br /> Manager was not able to provide an electronic copy. Emergency response plans and procedures, chemical inventor <br /> and site map shall be readily available to personnel of the business or facility with responsibilities for emergency <br /> response or training.
