Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />June 20, 2018 <br /> Facility Address: <br /> 16555 VON SOSTEN RD, TRACY <br /> Facility Name: <br /> LAMMERSVILLE SCHOOL <br />RemarksItem # <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />The site map was not complete as part of the business plan. the site map is missing emergency shut off loation for <br />the diesel and an evacuation stagings area location. A site map shall contain a north orientation, loading areas, <br />internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation <br />staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map <br />element is not applicable for your facility then list it on the map and label as “NA". Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, <br />and submit to the EHD for approval. <br />This is a minor violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br />The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br />System (CERS) annually. The kast submital made was 12/22/2015. Please make a submittal for 2018. The <br />hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted <br />beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the <br />correct or updated information, and submit to the EHD for approval. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 7/20/2018. <br />Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials <br />information online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
