Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />July 24, 2018 <br /> Facility Address: <br /> 1048 W YOSEMITE AVE, MANTECA <br /> Facility Name: <br /> 7-ELEVEN INC #17647 <br />RemarksItem # <br /> 5 HSC 25506(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br />The Hazardous Materials Inventory forms for carbon dioxide was not complete. The carbon dioxide is currently <br />listed as 53 gallons and it should be listed as 500 pounds. This information must be current and submitted as part <br />of the business plan. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br />A site map was not complete as part of the business plan. The site map is missing the evacuation staging area <br />location. A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and <br />sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling <br />and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then <br />list it on the map and label as “NA". Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 8/23/2018 . <br />Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials <br />information online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
