A site map shall contain a north orientation, loading <br />areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, <br />evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If <br />a site map element is not applicable for your facility then list it on the map and label as "NA". Immediately log into <br />the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated <br />information, and submit to the EHD for approval. <br />This is a minor violation. <br />9 <br />HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br />The Emergency Response and Training Plan element has not been submitted for 2018. The hazardous materials <br />inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of <br />the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated <br />information, and submit to the EHD for approval. <br />This is a Class II violation. <br />11 <br />HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br />No current proof of training available for inspection. The business plan shall include provisions for ensuring that <br />appropriate personnel receive initial and annual refresher training. This training shall be documented electronically <br />or by hard copy and shall be made available for a minimum of three years. Immediately provide employee training <br />for appropriate personnel and submit a copy of the training records to the EHD. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 2/17/2018. <br />Please be aware as of January 1, 2013, all businesses are required to submit all hazardous materials <br />information online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
