The following is an itemized list of hazardous waste violations that have not been <br />addressed for GEMCO RIPON TRUCK PLAZA INC as of June21, 2018. <br />Open violations from February 20, 2018 inspection <br />Violation #101 - Managed a hazardous waste without an identification number. <br />This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose <br />of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC <br />web site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. <br />Immediately reactivate your EPA ID number and submit evidence to the EHD. <br />Violation #110 - Failed to keep signed copy of manifests from the designated facility for three years. <br />Copies of uniform manifests for the disposal of hazardous waste for 2015, 2016, and 2017 were not found on site. <br />Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br />used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br />Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit <br />copies to the EHD. <br />Violation #111 - Failed to comply with uniform hazardous waste manifest exception requirements. <br />No manifests were found on site. Hazardous waste generators shall retain copies of all manifests for three years or <br />until a copy signed off by the destination facility is received. The signed off copy shall be retained and made readily <br />available for review for three years. If the generator did not receive a copy of the manifest with the handwritten <br />signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the <br />date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing <br />manifest, with some indication that the generator has not received confirmation of delivery. This information shall <br />be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. <br />Submit to the EHD a copy of the signed off copy of the manifests or a copy of the information submitted to DTSC. <br />Violation #113 - Failed to keep copies of consolidated manifesting receipts for three years. <br />Copies of hazardous waste disposal records for fuel filters from 2015, 2016, and 2017nwere not found on site. <br />Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br />used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br />Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit <br />copies to the EHD. <br />Violation #608 - Failed to properly manage used oil and fuel filters. <br />Bills of lading for the recycling of used oil/fuel filters for the last three years were not found on site. A copy of each <br />bill of lading must be kept on site for at least three years. Immediately locate a copy of all bills of lading for used <br />oil/fuel filters for the last three years, maintain them on site, and submit copies to the EHD. <br />Page 1 of 1 <br />
