The following is an itemized list of hazardous waste violations that have not been addressed for GEMCO RIPON TRUCK PLAZA INC as of June21, 2018. Open violations from February 20, 2018 inspection Violation #101 - Managed a hazardous waste without an identification number. This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC web site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. Immediately reactivate your EPA ID number and submit evidence to the EHD. Violation #110 - Failed to keep signed copy of manifests from the designated facility for three years. Copies of uniform manifests for the disposal of hazardous waste for 2015, 2016, and 2017 were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. Violation #111 - Failed to comply with uniform hazardous waste manifest exception requirements. No manifests were found on site. Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submit to the EHD a copy of the signed off copy of the manifests or a copy of the information submitted to DTSC. Violation #113 - Failed to keep copies of consolidated manifesting receipts for three years. Copies of hazardous waste disposal records for fuel filters from 2015, 2016, and 2017nwere not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. Violation #608 - Failed to properly manage used oil and fuel filters. Bills of lading for the recycling of used oil/fuel filters for the last three years were not found on site. A copy of each bill of lading must be kept on site for at least three years.
