The following is an itemized list of hazardous materials violations that have not been <br /> addressed for ROUND TABLE PIZZA as of March 30, 2015. <br /> Open violations from September 25, 2014 inspection <br /> Violation#1 -Failed to establish and implement a hazardous materials business plan. <br /> A Hazardous Materials Business Plan has not been established or implemented. A Hazardous Materials Business <br /> Plan shall be established and implemented for emergency response to a release or threatened release of a hazardot <br /> material when storing hazardous materials at or above the thresholds quantities of 55 gallons for materials that are <br /> liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperature and pressure. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> required information, and submit for review by the EHD. <br /> This is a Class II violation. <br /> Violation#2 -Failed to electronically submit a business plan. <br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System <br /> (CERS). As of January 1, 2013, all businesses were required to submit all business plans electronically. Immediatel <br /> log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for <br /> approval. <br /> This is a Class II violation. <br /> Violation#6 -Failed to complete and/or submit the Business Activities or Owner/Operator Identification <br /> page. <br /> The Business Owner/Operator Identification form was not submitted to CERS . This information must be current ar <br /> submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) <br /> at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> Violation#7 - Failed to complete and/or submit hazardous materials inventory for all reportable materials <br /> on site. <br /> The Hazardous Materials Inventory form for 450 Pounds of Carbon Dioxide was not submitted to CERS. This <br /> information must be current and submitted as part of the business plan. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and <br /> submit to the EHD for approval. <br /> This is a minor violation. <br /> Violation#10 -Failed to complete and/or submit a site map with all required content. <br /> A site map was not submitted as part of the business plan. A site map shall contain a north orientation, loading <br /> areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, <br /> evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> Page 1 of 2 <br />
