SANJ 0 A U I Environmental Health Department <br /> 0U NT <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> AVIVA PERFORMANCE 1414 W FREMONT ST, STOCKTON July 26, 2018 <br /> Item# Remarks <br /> 5 HSC 25506(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> Greater than 55 gallons of new motor oil, diesel exhaust fluid and antifreeze were observed and the material has not <br /> been reported. Any material that meets or exceeds the reportable quantity shall be reported in the facility's business <br /> plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add <br /> the materials to the hazardous material inventory, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed or submitted as part of the business plan. A site map shall contain a north orientation, <br /> loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, <br /> evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a <br /> site map element is not applicable for your facility then list it on the map and label as "NA". Immediately log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection, July 26, 2018. The EHD ha: <br /> written the complete report which replaces the initial checklist. Complete and submit a copy of the Return to <br /> Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or <br /> will be taken for each violation, and any supporting paperwork, within 30 days of receiving the completed <br /> inspection report. <br /> Business has been at this location since July 1, 2018. <br /> Complete submittal due no later than August 1, 2018. <br /> Assistance with CERS access and partial submittal provided at the time of inspection. <br /> Please be aware as of January 1, 2013, all businesses were required to submit all hazardous materials informatio <br /> online to the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov.
