The following is an itemized list of hazardous materials violations that have not been <br /> addressed for CARLS JR RESTAURANT as of June 20, 2018. <br /> Open violations from May 16, 2018 inspection <br /> Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation #10 - Failed to submit revisions to business plan within 30 days of substantial change in <br /> operations. <br /> During the inspection, it was found that and the updated information has not been submitted. Within 30 days of one <br /> the following events, business shall electronically updatethe information submitted to the California Environmental <br /> Reporting System (CERS).- <br /> - <br /> CERS):-A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual <br /> refresher training. This training shall be documented electronically or by hard copy and shall be made available for <br /> a minimum of three years. Immediately provide employee training for appropriate personnel and submit a copy of <br /> the training records to the EHD. <br /> Violation #12 - Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br /> plan readily available for review. <br /> Page 2 of 2 <br />
