Immediately provide sufficient secondary containment for this and all other portable <br /> containers at this facility, or provide equivalence as allowed by CFR 112.7(a)(2). <br /> This is a Class II violation. <br /> 721 CFR 112.8(c)(11) Failed to locate mobile or portable containers to prevent discharge. <br /> The black steel 55 gallon drum of fuel contaminated test water was not located to prevent a discharge. Portable oil <br /> storage containers must be positioned or located to prevent a discharge and shall be furnished with a secondary <br /> means of containment sufficient to contain the capacity of the largest single container with sufficient freeboard to <br /> contain precipitation. Immediately relocate these and all other portable containers at this facility to prevent a <br /> discharge. <br /> This is a Class II violation. <br /> 726 CFR 112.8(d)(4) Failed to regularly inspect aboveground valves, piping, and appurtenances. <br /> No inspection records were found for aboveground valves, piping and appurtenances. All aboveground valves, piping, <br /> and appurtenances shall be regularly inspected. Immediately inspect aboveground valves, piping, and appurtenances <br /> and ensure inspections are done regularly, or provide equivalence as allowed by CFR 112.7(a). <br /> This is a Class II violation. <br /> 727 CFR 112.8(d)(4) Failed to conduct integrity and leak test on buried piping any time it is worked on. <br /> No integrity test results were found on site for buried piping. Buried piping shall be tested for integrity and leaks at the <br /> time of installation, modification, construction, relocation, or replacement. Immediately perform integrity and leak <br /> testing of this piping and ensure that piping is tested when required, or provide equivalence as allowed by CFR <br /> 112.7(a)(2). <br /> This is a Class II violation. <br /> 728 CFR 112.8(d)(5) Failed to adequately warn vehicles entering facility to protect piping and other transfer operations. <br /> No warnings were observed on site to warn vehicles of aboveground piping, containers, or other appurtenances. <br /> According to the plan, warning sites are posted at the aboveground tanks A warning must be provided to all vehicles <br /> entering the facility to be sure that no vehicle will endanger aboveground piping or other oil transfer operations. <br /> Immediately provide a warning in accordance with CFR Part 112 for all vehicles entering the facility, or provide <br /> equivalence as allowed by CFR 112.7(a)(2). <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Page 7of8 <br />
